POSITION SUMMARY

The Mustard Seed Edmonton is committed to helping people who are homeless transition toward and maintain sustainable and healthy lifestyles. The Housing Support Worker is a member of a team that provides housing and facilitates support services for 20 high-acuity persons experiencing multiple barriers, who require the support of Permanent Supportive Housing (PSH).

 

The main role of the Housing Support Worker will be to provide case management services, assist clients with daily living skills, connecting to community resources and other day to day tasks as needed. The Housing Support worker will work with the Housing team to provide ongoing addiction and mental health support. The Housing Support Worker will be working onsite at the residence which functions within a harm reduction model.

    (Week 1: 7:00–15:00 and Week 2: 15:00–23:00)

5. Formalize and implement advance care planning documents: personal directives, enduring power of attorney, wills and end of life wishes.
6. Provide resources to address the following: finances, forms (pre and post death), end of life services, legal contracts and private practice rehab practitioners.
7. Chart in progress notes.
8. Assist families / significant others with the grief process by participating in bereavement follow up programs.
9. Collaborates and liaise with other professional disciplines (community physicians, agencies and clergy) to enhance the continuity of care to residents, families, volunteers and employees.

Boys and Girls Clubs Club Volunteer

In School Mentor

In as little as one hour a week, you could change the life of a child forever. Bigs In School is an easy and rewarding way to get involved with a child. It allows volunteer mentors go into schools to spend an hour with a child once a week. One of the unique aspects of Bigs In Schools is that the time is guided by the 20-20-20 rule. Mentors and students spend 20 minutes getting to know each other or catching up, 20 minutes going over homework or reading, and the last 20 minutes participating in recreational activities. These are the times when our frontline workers spring into action. Work hours at Wood’s Homes are typically round-the-clock and the type of work we do requires quick thinking, confidence, forgiveness and persistence.

We are currently seeking 4 casual (front-line) School Support Counsellors I or II for our Specialized Learning Centres. Our Learning Centres (*in partnership with CBE) offer opportunities for achieving academic, social and emotional success to young people and families who have had previous unsuccessful school experiences and have lost or are at risk of losing their community school placement.

Starting Points:  Counselling support for students in seven Calgary schools, in partnership with Calgary Separate School District.

George Wood*:  Bowness campus school for youth in intensive/community-based residential care.

William Taylor*:  Day-treatment school for community youth, and youth in residential care, at Parkdale Campus.

Children’s Village (CVS)*:  A day-treatment community-based school in Renfrew for children 4-12 years who exhibit severe emotional and behavioural difficulties.

The School Support Counsellor is responsible for front-line care of young people admitted to Wood’s Homes school settings. The School Support Counsellor works collaboratively with various school personnel and liaises with the young person’s family and community.

William Taylor*:  Day-treatment school for community youth, and youth in residential care, at Parkdale Campus.

Children’s Village (CVS)*:  A day-treatment community-based school in Renfrew for children 4-12 years who exhibit severe emotional and behavioural difficulties.

The School Support Counsellor is responsible for front-line care of young people admitted to Wood’s Homes school settings. The School Support Counsellor works collaboratively with various school personnel and liaises with the young person’s family and community.

Youth and Family Support Worker - BASE CAMP

The Youth & Family Support Worker will assume collaborative responsibility for the management of individual cases with a Shift Supervisor and AHS Addiction Counselor.  Each worker is assigned to a key client and is responsible for the activities related to the admission, case/treatment plan and transition plan related to the youth.  In providing this care Youth & Family Support Workers must have a working knowledge of the policy manual and accreditation standards used by Enviros, and the policies used by AHS, with regard to the services provided to the youth and his/her family.  This is inclusive of the youth’s rights, services available to them and the program's responsibilities in providing services.

It is your responsibility to communicate and collaborate with other professionals (e.g. transition/intake workers, Shift Supervisors, AHS Addictions Counselors, Family Counsellors, probation officers, teachers, etc.) involved with your key client.  This means ensuring timely meetings occur to meet the goals and objectives of the case and/or treatment plan.

It is your responsibility to communicate with the family/support people for your key client.  This means ensuring timely communication, updates and planning to maintain support and a positive relationship with the family/support people.

It is your role to pass this information to the rest of your staff team.  This establishes a consistent environment for the youth in the program and on visits, and allows the program staff to work as a team.  

As a key worker, you are required to develop a relationship with the youth to assist them in meeting their goals while in the program.    In this relationship, you are required to meet with the youth on a weekly basis to establish and discuss these goals while in the program.  

While on shift at Enviros, it is your role to develop positive relationships with all youth who reside in the program.  As a Youth & Family Support Worker you set the stage for how conflicts will be resolved and are an example of how to speak and act respectfully to others.  It is also part of your role to deliver an effective treatment program and support youth in effecting change.  In order to assist youth in developing these skills you must be present with them and be involved with interacting with the youth whenever possible.  

Wilderness-based activities are a significant part of the Base Camp program.  Trips are taken with youth to increase the opportunities for experientially based learning and expose youth to a different therapeutic environment.  You will be expected to participate in and contribute to the trip (treatment planning, facilitation, debriefing, logistical preparation and trip planning, etc.), depending on your skills and abilities.

The Prairie Central FASD Association is looking to hire a temporary 1.0 FTE for the role of Executive Director.  The position is based out of Killam, AB and will require extensive travel.

The Role of the Executive Director is:

  • To ensure that the over-all direction, philosophy, guidelines, policies and programs of the Prairie Central FASD Association are carried out in a manner consistent with values, mission, vision and direction of the Prairie Central FASD Association and Council
  • Receive direction from, report to, and maintain regular communication and consultation with, the Prairie Central FASD Association Council
  • Collaborate and liase with the key stakeholders who are engaged with the Association, thereby assisting the Council in developing relevant annual work-plans based on shared information within the Association and specifically with key stakeholders
  • Ensure the delivery of comprehensive, accessible, and coordinated services across the geographic regions of the Association
  • Ensure the Association is aligned with funders’ expectations for reporting, including preparation and submission of required reports
  • Manage all external contracts of the Association
  • Supervise and manage programs of the Association
  • Manage and report on the finances of the Prairie Central FASD Association

 

 

Salary will be based on experience and qualifications

Start date: June 18, 2018 with flexibility

Currently, Bissell Centre is looking for the right individual to fill the position of Casual PSH Support Worker. The key function of this position is to work with adults who are living with a Fetal Alcohol Spectrum Disorder to support home stability and goal achievement through a client centered approach and wrap around services. The PSH Support Worker will act as a broker between the tenants and resources to enhance the tenant’s self-reliance within their home and community.

This position requires individuals who are willing to work on 24/7 rotating 10 hour shifts and weekends.  Individuals must have the ability to respond quickly to individuals in crisis.
