This is an exciting time to practice Social Work with Alberta Health Services! We are excited to be offering Restorative Care in the Town of Mayerthorpe. With this comes the opportunity to help build our team and services, offering a chance to truly make a difference. Restorative Care is an intensive rehabilitation program designed to help patients, particularly seniors, maintain and improve function. Restorative Care focuses on improving an individual’s physical function and mobility, with the goal being to return to their own home or transition to the most independent level of care possible. Restorative Care is a multidisciplinary care model that is intended to also improve health and psychosocial outcomes of the clients in order to reduce caregiver burden, and to adopt evidence-based clinical practice that has demonstrated an ability to improve functional independence for clients.

Qualifications:     • Completion of bachelor's degree in Social Work.
• Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Description:     As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
